A Team Summary Report can also be called an Evaluation Results Summary Report or a Multidisciplinary Team Evaluation Summary Report. It is the form that records the results of the tests the school is relying upon to determine if your child is eligible for special education.
The form could be used to record the results of your child's first evaluation when initially considering whether your child qualifies for special education.
The form could be used to record a mix of old and new test results for a 3-year re-evaluation for special education.
The form could be used if your child is struggling in some area of his/her education and you or the school asked for testing to confirm there are new needs that should be addressed in an IEP.
Sometimes the school or district chooses to combine this document with an Eligibility document and call it a "Team Evaluation Summary Report and Prior Written Notice of Eligibility Determination: (name of IDEA eligibility category)"
Even within a district, schools may choose which format they use.
